Position: LPN-GOS EOW 7A-7P Sat/Sun Part-Time

Position Title and Reporting

LICENSED PRACTICAL NURSE (LPN) – Reports to Director of Nursing (DON). Classification:
Non‑exempt.

Position Summary

The LPN provides licensed nursing services to assisted living residents, supervising staff, and fostering a warm, caring environment to ensure the highest quality of living.

Essential

Job Duties
  • This position is a Safety Sensitive Position. The essential functions, physical demands and mental competencies require continuous alertness and safe conduct.
  • Administer and document medication and treatments per physician orders, accurately recording all care provided.
  • Comply with Adult Residential Care Provider Level 3 Rules, Regulations, and Louisiana State Board of Nursing Regulations.
  • Identify resident problems and emergency situations, initiating life‑saving measures in the absence of a physician.
  • Encourage, remind and assist residents to attend community activities.
  • Provide direct nursing care and clinically supervise nursing assistants on your unit, ensuring efficient performance.
  • Contribute to continuity of care and effective communication by reporting care provided and changes in resident condition to the primary nurse or designee in a timely manner; may participate in routine activities of resident care including daily living and basic nursing procedures.
  • Assist residents with lifts, moves and transports, using proper body mechanics or lifting devices to prevent accidents.
  • Respond promptly and courteously to inquiries from residents, visitors and other personnel within established time frames and policy.
  • Be knowledgeable of individualized care plans, providing support according to each resident’s plan and contributing to the care‑planning process with observations of resident needs and preferences.
  • Communicate tactfully with residents, visitors, families, peers and supervisors to promote a homelike environment.
  • Understand all aspects of residents’ rights, including the right to be free of restraints and abuse; promptly report incidents or evidence of abuse or rights violations to the charge nurse or administrative staff.
  • Perform all responsibilities in accordance with prescribed safety and infection control procedures, including hand washing, use of disposable gloves, and proper disposal of soiled materials. Maintain skin integrity of residents, preventing pressure ulcers, skin tears, and other damage by turning, repositioning, and moisturizing as needed.
  • Perform tasks assigned by the charge nurse, such as checking vital signs, weighing residents, applying creams/ointments, and collecting specimens.

Qualifications
  • High school diploma or GED is required.
  • Current Nursing License in good standing with the Louisiana State Board of Practical Nursing.
  • Must meet all healthcare requirements, including a Tuberculosis skin test.
  • Must be in positive standing with all healthcare agencies, including the Direct Services Worker Registry.
  • Certified in cardiac pulmonary resuscitation and adult first aid within ninety (90) days of hire.
  • One‑year prior nursing experience preferred.
  • Assisted Living Facility experience preferred.
Mental Demands

Work requires adherence to precise procedures and standards involving a high degree of accuracy in observing, recording, and reporting data. The employee must frequently communicate and exchange information accurately.

Physical Demands

This position is very active and requires frequent standing, walking, bending, stooping, lifting, handling, pushing, pulling and occasional carrying. The employee must lift or move objects and patients weighing over 50 pounds. Occasional use of feet and legs is needed to release and lock brakes of wheelchairs and beds. The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job.
