Patient Care Manager – Registered Nurse (RN)
The Home Health Patient Care Manager is responsible for overall supervision and coordination of clinical services and oversees an interdisciplinary team to provide high‑quality care to home health patients.
Primary Responsibilities- Provide clinical services within the scope of practice, in accordance with the plan of care and coordination with other team members.
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start‑of‑care visits.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continuously assessed and care is individualized, appropriate, and in accordance with physician orders.
- Review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations as appropriate.
- Function in any home situation regardless of client characteristics.
- Current unrestricted RN licensure in the state of practice.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification or ability to complete within 90 days of hire.
- Home care experience.
Pay ranges from $76,228 to $114,342 annually for full‑time positions. Additional benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility). Pay is based on market, education, experience, and certifications.
