Louisiana Hospice and Palliative Care, part of LHC Group and Optum, is seeking an RN Case Manager to deliver the hospice plan of care for each patient. You will evaluate clinical performance and adjust care to improve quality and outcomes, in collaboration with the care team and patients’ families.
You will determine eligibility within 48 hours of referral, identify patient and family needs, initiate and coordinate the care plan, document progress, and support self-care by families.
