Center Well Home Health, a Humana subsidiary, seeks a Home Health LPN to provide personalized, one-on-one nursing in patients' homes. You will implement care plans, educate families, maintain records, and coordinate with the care team to support recovery and independence.
You'll work under an RN, drive to patient homes, and participate in a TB screening and Humana's driver safety program. Prior LPN/LVN experience preferred; valid license and CPR required.
