DESCRIPTION

The SUD Jail Diversion Peer Support Specialist will act as a support and resource for adults who are in the correctional setting and preparing for exit (or recently exited) the criminal justice system related to substance abuse and behavioral health treatment services. The SUD Jail Diversion Peer Support Specialist will act as a key member of the treatment team by working with consumers one-on-one to support them in achieving goals and connecting consumers to community resources.

The SUD Jail Diversion Peer Support Specialist will support consumer skills in order to improve their overall functioning and quality of life. The peer will work with both women and men incarcerated at the Monroe County Jail. This position works in tandem with the SUD Jail Diversion clinician assigned to the treatment team. This position reports to the Jail Diversion SUD Clinical Services Supervisor/Coordinator.

  • Build and foster relationships with the consumer, community partners and other key supports.
  • Actively engage as part of the treatment team to meet consumer needs, complete all required documentation and paperwork.
  • Prepare the consumer to fully participate in their own meetings and treatment processes.
  • Teach self-advocacy, problem-solving and independent living skills.
  • Provide support during "rough" or "crisis" times.
  • Empower the consumer to build understanding in others about their unique mental health needs.
  • In collaboration with the consumer, work with other internal and external partners (i.e., schools, faith community, other human services organizations) to develop natural supports to eliminate barriers to services, enhance existing community services or develop new services based on the needs of the consumer.
  • Provide consultation and education to consumer and use problem-solving techniques with the objective of assisting consumer in improving or maintaining functioning.
  • Serve as a role model and active participant in supporting consumer in meeting their treatment plan goals.
  • Conducts consumer check-ins as necessary and appropriate.
  • Assists in facilitating community referrals to resources identified by the treatment team, consumer and family.
  • Provide transportation for consumer to resources, treatment and other key activities for successful community re-entry
  • Attends staff, team and department meetings.

Performs other related duties as assigned

  • Maintain working knowledge of trends in the mental health field through participation in recovery based trainings, conferences, reading and committee/meetings both on a local, affiliate, state or national level.
  • Follow oral and written direction completing tasks as assigned by due dates.
  • Maintain working knowledge of software applications used in performance of daily duties.

  • Complies with and maintains competence with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, the Michigan Department of Health and Human (MDHHS), Joint Commission, and any applicable funding sources.
  • Understands, adheres and maintains competence with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA's, Management of Information, Finance-Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Environment and Provision of Care Policies.
