** you’ll contribute to our success in the following ways:
* Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
* Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
* Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
* Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
* Document medication regimens and maintain accurate, up-to-date medication profiles.
* Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
* Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.  
** What is Required?
*** High School Diploma or GED equivalent required
* 1 year of experience as a Hospice Aide or Nursing Assistant is required
* State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
* Experience in a home health environment is
* preferred
