Concierge Home Care

We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide exceptional in-home clinical oversight. This field-based role is ideal for an organized, independent RN who enjoys building meaningful relationships with clients while delivering personalized, high-quality care in the comfort of their homes.

• Conduct comprehensive in-home nursing assessments
• Develop, implement, and oversee individualized plans of care
• Obtain and maintain physician orders in compliance with New Jersey regulations
• Monitor client conditions and promptly communicate significant changes
• Complete accurate and timely clinical documentation
• Perform Home Health Aide supervisory visits and ensure quality of care
• Provide clinical guidance and support to Home Health Aides and Nurses
• Promote client safety, independence, comfort, and overall well-being

• Active New Jersey Registered Nurse (RN) license
• Minimum of 2 years of recent RN experience in acute care, home care, or case management
• Recent experience with:
  • Ventilator and tracheostomy care
  • G-tube/PEG tube management
  • IV infusions
  • Wound care
• Current BLS Certification
• Current physical examination (within the past year)
• Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable
• MMR immunization documentation
• Valid driver's license and reliable transportation
• Authorized to work in the United States

• RN Assessment: $150 per visit
• RN Aide Supervision: $125 per visit
• Hourly Clinical Visits: $55 per hour
• Weekday On-Call: $50 per day (Monday-Friday, 5:00 PM-9:00 AM)
• Weekend On-Call: $110 per day (Saturday 8:00 AM-Monday 9:00 AM)
