- About Facility:
- Home Health - 1423 E Franklin Street Suite E Monroe NC 28112
- Radius rule: Traveler's permanent address must be 50 miles outside of home unit
- Department Information:
- Teammates are required to complete 6 visits a day (routine visits, discharge, ROC, recert, reassessment)
- Minimum Years of Experience required? 1 year Home Health experience
- Will you accept a first-time traveler? Yes, if seasoned to home health
- Patient Types/Common Diagnoses:
- Chronic diseases, wound care, IV, pleurx, wound vac
- There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
- Teammates will travel throughout the below areas:
- Small area of Charlotte, Matthews, Indian Trail, Waxhaw, Monroe , Wingate, Polkton and Wadesboro and cities in between
- Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
- Driver's License and proof of Insurance
- BLS
- Skills required:
- Homecare Homebase experience is required (no exceptions), IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS, case management (ability to oversee/episode management up to 25-30 patients at once)
- Support within the Department:
- Departmental leadership
- Schedular
- Clinical Manager
- Clinical Supervisor
- Peers/Clinical Team
- Technology/Equipment:
- EPIC Rover - view only
- Focura - will upload discharge instructions and other important docs such as wound pictures
- Samsung note 9 phones
- EMR: HCHB (Homecare Homebase) - experience required
- Orientation (facility and unit):
- NEO
- 5 days orientation
- Scheduling:
- Weekend rotation: none, unless willing and interested in working weekends
- Call is not required, but may be available if interested
- Holiday Expectations:
Travelers will work 75% of holidays - RTO:
Manager approval - Schedule cycle: every 13 weeks
