Position Summary

RN Case Manager in the home or community setting providing intermittent skilled nursing services, directing and supervising hospice aides, and coordinating care for patients and families.

Responsibilities
  • Assume primary responsibility for the patient/family caseload, including assessing, planning, implementing, and evaluating care needs during on‑call hours in collaboration with the triage team.
  • Report on‑call activities to clinical leadership and Branch RN Case Managers at the end of each shift.
  • Maintain and complete appropriate care coordination notes, visit notes, and other pertinent clinical documentation.
  • Assist with, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays.
  • Attend interdisciplinary team meetings during on‑call periods for collaboration, consultation, and evaluation of patient/family needs.
  • Under physician orders, admit patients eligible for hospice services in collaboration with the triage team.
  • Report patient status and needs for additional disciplines to clinical leadership, attending physician, and hospice physician.
  • Develop patient care plans that address identified problems, nursing goals, and mutually agreed goals with patients during on‑call hours, weekends, and holidays.
  • Ensure the use of the 4

    Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide age‑friendly care.
Qualifications
  • Current unrestricted RN licensure in the state of practice.
  • Valid driver’s license and proof of auto insurance.
  • Minimum two years of RN experience.
  • Preferred: at least two years of Hospice/Palliative Nursing experience.

Schedule

Mon‑Fri 8am‑5pm. On call as needed.
