Support Coordinator (QP)

InReach has an exciting opportunity to join our team as a full-time Support Coordinator (Qualified Professional) in the Monroe area that is responsible to develop and coordinate Plans of Care for individuals with Intellectual/Developmental Disabilities. Will include meeting with IDD individuals and families to address needs and concerns; must also be willing to work cooperatively with community partners as well as staff from other InReach departments.

Schedule:

Monday-Friday 8:30am to 4:30pm
* hours may vary depending on customer needs; nights and weekends and on call may be necessary.

146 Winchester Ave, Monroe, NC 28110

Pay Rate:

$50,000 - $55,000 based on experience

Responsibilities may include (but not limited to):
  • Supervising Community Outreach staff in the implementation of supports to individuals served.
  • Monitoring, managing, and facilitating operations on a day-to-day basis.
  • Responsible to complete all documentation in a timely manner.
  • Implementation and review of customers Plan of Care.
  • Plan and oversee work schedule, approve payroll hours.
  • Provide one‑on‑one supervision as needed.
  • Share on‑call service for emergencies.
Qualifications:
  • 4-year degree in the human services field
  • 2 years of supervised direct experience working with the IDD population
  • 1-2 years of management/supervisory experience preferred
  • Innovations Waiver and 1915i services experience preferred
  • Possess a valid driver's license
  • Access to reliable transportation 24-hours a day
  • Safe driving record.
