Evolve Therapy Services is seeking a Speech Language Pathologist to provide in-house therapy services at a Skilled Nursing Facility in Freeport, IL. This PRN role focuses on delivering quality therapy and coordinating care with staff to meet CMS standards.
You will evaluate residents, develop treatment plans with short- and long-term goals, and document findings in the Net Health EMR. Must have CCC and active SLP license in state(s) of practice.
