Physical Therapist ​/ Therapy (PT) - Must Be Licensed

Responsibilities

The Physical Therapist / PT is responsible for evaluating referred patients requiring rehabilitation services, providing direct patient care, assessing medical condition, functional capabilities, limitations, and potential for rehabilitation, establishing and administering treatment programs with goals determined by the patient’s capacity and tolerance under physician direction, setting realistic goals, documenting patient condition, and educating patients and families about appropriate physical therapy methods.

• Graduate from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each patient.
• Provide skilled physical therapy services and interventions per physician orders.
• Assess, plan, evaluate, and modify care to meet therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
