Serenity Hospice is expanding into Western Pennsylvania and we’re looking for a compassionate and dedicated Hospice Social Worker to join our growing Monroeville team. This is an exciting opportunity to help build a new territory while providing meaningful support to patients and families during life’s most important moments.

• Provide emotional, psychosocial, and practical support to hospice patients and their families

• Conduct psychosocial assessments and contribute to individualized care plans

• Assist families with community resources, advance care planning, and care transitions

• Provide grief and bereavement support and education

• Collaborate with nurses, chaplains, aides, and leadership as part of the interdisciplinary team

• Help establish Serenity Hospice’s presence in the Western PA community

• BSW or MSW from an accredited social work program

• Healthcare experience required; hospice experience strongly preferred

• Strong communication, assessment, and relationship-building skills

• Ability to work independently and as part of an interdisciplinary team

• Valid driver’s license and reliable transportation for field visits
