Position SummaryThe RN Case Manager is responsible for coordinating and managing the care of hospice patients, ensuring the highest quality of care while serving as the primary clinical liaison for patients, families, physicians, and the interdisciplinary team.
The ideal candidate is compassionate, organized, clinically skilled, and committed to delivering outstanding end-of-life care.
QualificationsCurrent Pennsylvania Registered Nurse (RN) license
Minimum of one (1) year hospice experience as an RN Case Manager required
Strong clinical assessment and care coordination skills
Excellent communication and interpersonal abilities
Ability to work independently while collaborating effectively with an interdisciplinary team
Valid driver's license and reliable transportation
