Monroeville, PA | Full-Time | $90,000 per Year

Serenity Hospice PA is a growing and rising hospice organization seeking an experienced Registered Nurse Case Manager to join our Monroeville team. Responsibilities
  • Coordinate and manage individualized plans of care for hospice patients

  • Complete comprehensive nursing assessments and ongoing evaluations

  • Provide skilled nursing care, symptom management, and patient education

  • Collaborate with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members

  • Educate and support patients and families throughout the end-of-life journey

  • Maintain accurate and timely clinical documentation

  • Participate in interdisciplinary team meetings

  • Help ensure quality, compassionate, and compliant hospice care

Qualifications
  • Current Pennsylvania Registered Nurse license

  • Hospice, home health, palliative care, or case-management experience preferred

  • Strong clinical assessment, communication, and organizational skills

  • Valid driver’s license, reliable transportation, and automobile insurance

  • Compassionate, dependable, and comfortable working independently in the community

  • Commitment to providing exceptional end-of-life care
