Outreach and Enrollment Specialist

Part-time (24 hours/week) pro-rated benefits-eligible

Position Summary:

Under the general direction of the Outreach and Enrollment Supervisor, the Outreach and Enrollment Specialist supports an integrated care team by helping patients access healthcare and related services, including enrollment in Marketplace, Medicare, and Medicaid programs. This position conducts community outreach to improve access to primary and preventive care by assisting residents within the Blacktail Health service area with navigation of insurance enrollment processes.

The Specialist collaborates with internal and external service providers to coordinate resources that support patient success. The Specialist also assists the Outreach and Enrollment Supervisor with additional duties as assigned.

Position Requirements :
  • Education:

    High School Diploma or equivalent required;
    Degree in social work, public health, human services, or related field preferred, educational equivalent may be considered with 2+ years of related experience.
  • Licenses:
    Valid state-issued driver’s license required;
    Montana Certified Application Counselor license or ability to obtain within six months; BLS CPR certification or ability to obtain within 30 day.
  • Experience:

    Prior experience in a healthcare setting or care management role preferred; experience with health insurance administration and enrollment highly preferred; experience serving a diverse population including those who are vulnerable and/or may have high needs preferred; previous EHR experience preferred.

Essential Functions, Roles, and Responsibilities:
Outreach and Enrollment:
  • Serve as a central resource for community questions and referrals for both consumers and providers, building relationships with community resources, public health, schools, churches, and other social service organizations to identify, refer, and develop resources that remove consumer barriers to accessing needed health and social services.
  • Assist eligible clients to apply for health insurance plans, providing necessary follow-up, tracking and data collection to ensure continuous enrollment.
  • Develop and maintain a tracking and follow-up system for applications to ensure that all aspects of the process, from application through post-enrollment, have been completed and that the client is able to receive appropriate services.
  • Maintain statistics/data on contacts; provide reporting, identify needs, problems, and service gaps; track and tabulate contacts, application submission and post-enrollment follow-up.
  • Initiate post-enrollment support activities and record information required for monthly and quarterly reports.
  • Attend appropriate community or networking meetings to facilitate outreach and gathering of information to increase consumer access to a permanent medical home.
  • Assist in developing educational materials to distribute through local community groups.
  • Assist in organizing and holding external education and enrollment events to increase access to our services in the community.
  • Travel as needed to assist with outreach and enrollment activities.
  • Coordinate with other departments for marketing campaigns to increase awareness in the community of Open Enrollment, deadlines, and opportunities for health insurance enrollment.
  • Support and contribute to effective safety and risk management efforts by adhering to established policies and procedures, maintaining a safe environment, promoting accident prevention, and identifying and reporting potential liabilities.
