Position Overview A community health center is seeking a Family Nurse Practitioner (FNP) or Certified Physician Assistant (PA-C) to join a multidisciplinary team providing comprehensive primary care services in a rural community setting.

This opportunity offers the ability to care for patients across the lifespan while working within an integrated care model that includes primary care, behavioral health, pharmacy, and dental services. Position Highlights Full-time outpatient position Integrated care model including:
