Speech Language Pathologist, Home Health

• Promote Elara Caring's philosophy, mission statement, and administrative policies to ensure quality of care
• Evaluate patients to determine rehabilitation needs and potential for achievement
• Develop and implement individualized plans of care
• Admit patients and assess speech-language therapy and other needs according to OASIS and PPS guidelines
• Perform initial therapy evaluations and re-evaluate patients during visits
• Conduct follow-up assessments according to OASIS and PPS guidelines
• Initiate patient safety plans using patient, family, and community resources
• Communicate significant findings, problems, and changes in condition to supervisors, physicians, and care team members
• Participate in Plan of Care development, implementation, continuity of care, and discharge planning
• Verify the Plan of Care before each visit and provide care according to physician orders and standards
• Write physician orders for additional visits and Plan of Care changes according to agency policy
• Incorporate patient care goals into therapy and document them in therapy notes
• Initiate and revise Plans of Care in response to patient care issues
• Provide direct Speech-Language Pathology services
• Select diagnostic and therapeutic materials appropriate to patient needs
• Instruct patients and caregivers in home therapy programs
• Maintain communication and coordination with physicians, staff, and other disciplines
• Ensure therapy notes, supply logs, and visit logs are accurate and submitted according to policy

• Master's degree from an accredited educational program in Language Pathology
• Current, unrestricted SLP license in the state of work
• Minimum one (1) year of experience as an SLP in a clinical care setting
• Able and willing to travel within the branch/office coverage area
• Able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Dependable vehicle
• Valid driver's license
• Current auto insurance in accordance with state laws
• Able and willing to travel 50%
• Ability to work in a physically demanding, high-stress environment
• Full range of body motion, including handling, lifting, and transferring patients
• Potentially able to work irregular hours, including call hours if applicable

Demonstrates expertise in Speech-Language Pathology, including the development and implementation of individualized care plans, patient evaluations, and adherence to OASIS and PPS guidelines. Strong communication and coordination skills with healthcare teams are essential for effective patient management and safety.
