Role Description

The Physical Therapist Assistant role at Eden Health is a full-time, on-site position located in Libby, MT. The Physical Therapist Assistant will support licensed Physical Therapists by implementing treatment plans, assisting with therapeutic exercises, and monitoring patient progress in home and community settings. Day-to-day responsibilities include providing hands-on care, maintaining accurate clinical documentation, communicating changes in patient status to the supervising Physical Therapist, and collaborating with the interdisciplinary care team.

The role also involves educating patients and caregivers on exercises, safety, and home programs to promote functional independence and quality of life.

Qualifications
  • Demonstrated skills in Physical Therapy and Rehabilitation, including implementing treatment plans and assisting with therapeutic interventions.
  • Ability to deliver Quality Patient Care and Patient Care that is safe, compassionate, and aligned with established clinical standards.
  • Experience or coursework in Orthopedics, particularly in supporting patients with musculoskeletal conditions and functional mobility needs.
  • Active state license or certification as a Physical Therapist Assistant (PTA) in Montana, or eligibility to obtain it prior to start date.
  • Knowledge of home health or hospice care practices and familiarity with interdisciplinary team collaboration.
  • Strong communication, documentation, and time-management skills, with the ability to work independently in patients’ homes.
  • Capacity to meet the physical demands of the role, including lifting, transferring, and supporting patients safely.
  • Valid driver’s license, reliable transportation, and ability to travel locally to patient homes as required.
