- Conduct comprehensive OT assessments to identify functional limitations and rehab needs for residents in a long-term care setting.
- Develop and implement individualized treatment plans aimed at improving activities of daily living, mobility, cognition, and overall independence.
- Administer evidence-based therapeutic interventions and make recommendations for adaptive equipment and environmental modifications.
- Monitor progress, adjust treatment plans as needed, and document outcomes clearly in resident records.
- Collaborate with an interprofessional team (nurses, physical therapy, speech-language pathology, social work) to coordinate care and support discharge planning when appropriate.
- Ensure compliance with regulatory standards, facility policies, and patient safety and privacy requirements.
- Availability for a 13-week travel assignment beginning 09/14/2026
- Willingness to work a Monday–Friday schedule in a long-term care setting
- Ability to document clearly and communicate progress with the interdisciplinary team
