Responsibilities
  • Conduct comprehensive OT assessments to identify functional limitations and rehab needs for residents in a long-term care setting.
  • Develop and implement individualized treatment plans aimed at improving activities of daily living, mobility, cognition, and overall independence.
  • Administer evidence-based therapeutic interventions and make recommendations for adaptive equipment and environmental modifications.
  • Monitor progress, adjust treatment plans as needed, and document outcomes clearly in resident records.
  • Collaborate with an interprofessional team (nurses, physical therapy, speech-language pathology, social work) to coordinate care and support discharge planning when appropriate.
  • Ensure compliance with regulatory standards, facility policies, and patient safety and privacy requirements.
