NP​/PA for Senior focused Primary Care clinic in Falls, MT

Great opportunity for a Nurse Practitioner or Physician Assistant to live and work in beautiful Great Falls, MT for a fast growing organization delivering care to older adults in access starved communities.

Board Certified Physician Assistant (PA) or Nurse Practitioner (NP) - FNP, ANP, ACNP or AGNP

Must have 3+ yrs exp in Primary Care and independent patient care

Will be focused on outcomes and not volume - prioritize quality over quantity

Manage a patient panel, supported by the MA/LPN's, Front Office Coordinator, Clinic Manager, Medical Director, and additional teammates

Full scope of primary care services including well acute, and chronic conditions

Perform Annual Wellness Visits and Health Risk Assessments

Medicare Advantage

Must be skilled with HCC coding
