- Conduct patient assessments, triage, screenings, and care coordination in collaboration with providers.
- Administer medications, treatments, immunizations, and injections per provider orders.
- Assist with exams and procedures; perform wound care, specimen collection, and lab testing.
- Monitor patient responses and report critical values promptly.
- Respond to medical emergencies and provide appropriate interventions.
- Communicate with patients, families, providers, and care teams to ensure continuity of care.
- Educate patients and families on health conditions, treatment plans, preventative care, and wellness.
- Identify health literacy needs and provide appropriate education and materials.
- Maintain accurate, timely documentation in the electronic medical record.
- Support audits, quality improvement initiatives, and regulatory requirements (PCMH, UDS).
- Participate in performance optimization and risk management activities.
- Perform medical assistant duties when needed and maintain competence.

Required Qualifications
- Nursing degree from an accredited nursing program.
- Current nursing license in the state of Montana.
- BLS certification (or ability to obtain upon hire).
- Valid Driver's License.
- Registered Nurse (RN).
- Two (2) years of nursing experience.
- Experience in a medical clinic or primary care setting.
- Experience working with diverse populations.
- Strong communication and interpersonal skills.
- Compassionate, patient-focused approach.
- Team collaboration and consultation skills.
- Sound clinical judgment and critical thinking.
- Professional discretion and ethical practice.
- Effective time management and adaptability.
