• Conduct patient assessments, triage, screenings, and care coordination in collaboration with providers.
• Administer medications, treatments, immunizations, and injections per provider orders.
• Assist with exams and procedures; perform wound care, specimen collection, and lab testing.
• Monitor patient responses and report critical values promptly.
• Respond to medical emergencies and provide appropriate interventions.

• Communicate with patients, families, providers, and care teams to ensure continuity of care.
• Educate patients and families on health conditions, treatment plans, preventative care, and wellness.
• Identify health literacy needs and provide appropriate education and materials.

• Maintain accurate, timely documentation in the electronic medical record.
• Support audits, quality improvement initiatives, and regulatory requirements (PCMH, UDS).
• Participate in performance optimization and risk management activities.

• Perform medical assistant duties when needed and maintain competence.

• Nursing degree from an accredited nursing program.
• Current nursing license in the state of Montana.
• BLS certification (or ability to obtain upon hire).
• Valid Driver's License.

• Registered Nurse (RN).
• Two (2) years of nursing experience.
• Experience in a medical clinic or primary care setting.
• Experience working with diverse populations.
