Work Unit Overview:

The MDS Coordinator is responsible forcoordinating and completing all require Resident Assessments Instruments and insure accuracy of information. Notify Inter Disciplinary Team (IDT) of any barriers to the timely completions of all residents Assessment Instruments. Coordinate and facilitate Resident Care Plan Conferences on a quarterly basis for all residents ensuring that the resident and the responsible party receive a written notification of the meeting specifying date and time and place of meeting.

Participate in the development of a comprehensive individualized plan of care for each resident. Ensure that the plans are completed, updated and accelerated to reflect an interdisciplinary approach to the residents changing needs.

Job Summary:

Review of each Medicare resident's case on the day to admission to ensure appropriateness of Medicare coverage. Active interaction with IDT on a weekly basis to evaluate resident's status and needs relative to Medicare skilled needs and potential for RUG category changes. Conducts review via chart audit to ensure the residents receive the care and services the they need in timely manner, change of condition RAls are initiated when appropriate, chart documentation, including nursing and ancillary notes, care plans note reflects the need for care and service the resident receives.

Discharge planning proceeds smoothly relative to resident's progress and Medicare coverage status. Medicare coverage is discontinued when resident on longer meets the criteria.

Report routinely to the Director of Nursing regarding Medicare status. Conducts quarterly audits to include: MDS, Care Plans, Psychotropic and any other as needed. Ensures that the Physician Certification/Re-Certification for Medicare A residents are completed on time.

Subject to additional duties such as working on the floor if needed. Or, other duties as assigned by the DON or facility administration in accordance to facility needs including participation in Call Schedule.

Interviews residents, interested family members, legal representatives and significant others to obtain and update information needed to develop individualized plan of care, to accommodate individual needs and preferences and to protect and promote resident's rights.

Works cooperatively with members of the interdisciplinary team to develop, implement, and evaluate plane of care. Attend care conferences. Communicates resident concerns and responses to interventions to interdisciplinary team members and direct care staff.

Audits clinical records of resident on a regular basis to monitor resident responses to care and treatment and to identify concerns about resident's rights. Suggests approaches and methods of interacting with residents that maintain and enhance the person's dignity and individuality.

Assist direct care staff with behavioral assessments and development of alternative approaches the minimize or eliminate use of physical and chemical restraint.

Identifies ways to accommodate resident's choices, preferences and customary routines. Includes these approaches in plan of care and gives this information to direct staff.

Maintains ongoing contact with interested family members, legal representatives with consent of resident to inform them of changes in condition, discharge planning efforts, and to encourage family participation in developing a plan of care.

Actively participate in long term care survey process by instructing staff in matter of conduct and discloser, maintaining a presence at all times surveyors are on site and directing the timely collection of information required by the survey team. Demonstrates concerns for identified problems and undertakes corrective action while survey is in progress if appropriate. Gathers and presents supplemental documentation to avoid potential deficiencies.

Assist the DON to develop responses to survey report of needed.

Reports injury to self or other to the charge nurse immediately.

Maintains current skills and knowledge through continuing education. Applies information to job responsibilities.

Participates in fire and disaster drills. In the event of an emergency, carries out assigned duties to assure resident safety.

RN currently registered in the state of Montana

Work experience:

Previous work experience in long term care setting and is listed in a good standing with the Montana State of Professional licensing.

The job requires freedom from illegal use of drugs and freedom from use and effects of drugs and alcohol in the workplace.

Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care setting are ineligible for employment in this position.

Must pass a background check.

Working knowledge and ability to apply professional standards of MOS Coordinator practice in job situations. Able to establish criteria to assure that the service provided meet established standards of quality.
