Nurse (Case Management

Duties

Manages an assigned caseload of patients with chronic or recurring health conditions that require case management services.

Develops and implements individualized, patient-centered care plans in collaboration with patients, families, and the healthcare team.

Participates in discharge planning activities for assigned patients, including identifying discharge needs, coordinating post-discharge services, and providing patient and family education.

Coordinates care across providers, disciplines, and care settings to ensure continuity and prevent fragmentation of services;
Provides direct professional nursing care to assigned patients with chronic and recurring health conditions.
