General Statement of Function: Coordinate and manage program services for assigned OPARC participants by developing and implementing Person Centered Plans (PCP) and Individual Habilitation Service Plans (IHSP), conducting regular meetings and visits, monitoring progress, maintaining records, and providing counseling and advocacy.
Essential Functions:
Participant Services
- Manage a participant caseload of approximately 45 participants.
- Develop Person Centered Plans (PCP) and One-Page Descriptions or use the existing PCP to guide the direction of the program. Based on the PCP, develop the Individual Habilitation Service Plan (IHSP) and other assessment forms in collaboration with assigned participants and other support team members.
- Coordinate, gather information for, and document meetings on an annual, semi-annual, and quarterly basis. Hold meetings with participants and support team members to discuss and review progress on objectives.
- Schedule and conduct visits to observe and engage with assigned participants.
- Monitor progress notes of participants’ goals and make recommendations for changes to the goals as needed or changes to the quality of tracking where needed.
- Maintain participant records in accordance with OPARC, Department of Developmental Services (DDS) and Commission on Accreditation of Rehabilitation Facilities (CARF) standards.
- Complete a Special Incident Report(SIR), Incident Report (IR), Observation Report, and APS Report for incidents as required.
- Assist management with the response implementation to participant advocacy findings.
- Assist participants with concerns relating to their daily activities and needs. Provide counseling, advocacy, and act as a liaison between care providers, Regional Center staff, OPARC staff, and peers.
- Analyze and report on quality of participant service delivery to Management, as it relates to staff.
- Complete all Exit/Discharge paperwork for participants no longer receiving services. Conduct a 6-month follow-up on their status from the date of termination.
- Provide coverage to the assigned back-up case manager in his/her absence.
- Assist in the coordination, implementation, and direct support of activities and fundraisers for all participants.
- Conduct program intakes as assigned, make appropriate recommendations based on findings, and collect required paperwork and documents.
- Hold IDT Meetings as necessary to ensure the program is meeting the needs of the individual.
- Act as a liaison to care providers, staff, and Regional Center service coordinators regarding participant needs, wants, and desires.
- Transport participants as needed.
- Ensure the safety of all participants. Adhere to all safety regulations.
- Perform all other duties and responsibilities as assigned. - Adhere to the proper care and responsible use of Oparc issued property, tools, and equipment.
Employment Services (if applicable):
- Provide 1:1 training and assistance to participants as needed and/or assigned.
- Provide input to management as it relates to the appropriateness of participant employment activities.
- Upon intake, validate and complete work documentation (I9 & W4).
- Provide benefits counseling and resources as needed.
- Attend New Start Meetings to ensure all parties have the information needed for a successful start, to include the completion of an orientation checklist.
- Caseload comprised of participants from Supported Employment Groups (SE), Individual Placements (IP), Paid Internship (PIP), Adult Work Experience (AWE), and Tailored Day Services (TDS).
