Registered Nurse, In-Patient Case Manager Night and Weekends

Job Overview

The RN Case Manager will provide daily care coordination with concurrent review for patients in the hospital or Skilled Nursing Facilities (SNF) in person or telephonically. This position will also provide case management to patients who are admitted to the hospital and patients who may need to be enrolled in ambulatory case management.

The RN Case Manager is responsible for identifying California Children Services (CCS) cases, handling transfers, and performing retro reviews. The role functions as part of an interdisciplinary care team, coordinating care and collaborating with social work, ambulatory case management, regional medical directors, hospitalists, hospital nursing personnel, and clinic physicians.

The RN Case Manager will perform assessments and apply appropriate guidelines to ensure the patient receives the correct level of care.

• Graduation from an accredited nursing program.
• Current valid license as a Registered Nurse through the California Board of Registered Nursing; a bachelor’s degree in nursing or another health or human services field with the appropriate licensure is preferred.
• Experience in and willingness to be part of a multidisciplinary team.
• Experience with physically or mentally impaired adults and/or the geriatric population.
• Minimum of 3 years of RN experience in public health nursing, acute care, case management, and/or home health care.
• Minimum of 2 years of managed‑care experience in case management with a focus on inpatient and/or outpatient ambulatory care.
• Bilingual in English/Spanish is preferred.
• BLS certification is required.

Annual salary range: $95,352.78 – $. Actual salary offers are determined by factors such as budget, experience, skills, education, licensure, certifications, and other business considerations. The employee may also qualify for discretionary bonuses or incentives.
