Alta Med Health Services Corp. is seeking an LVN Case Manager to deliver daily care coordination for patients in hospitals or SNFs, with telephonic reviews and ambulatory follow-up in California.
You will collaborate with social work, ambulatory case management, regional medical directors, hospitalists and clinic physicians to ensure seamless transitions and CCS case identification. A valid LVN license and bilingual English/Spanish are preferred for this role.
