Physical Therapist Assistant – Home Health (Salinas & South Monterey County)

Full‑time / Per‑Diem position at Choice Home Health – Monterey, CA. Hourly pay: $36.00–$48.00.

• Provide physical therapy assistance under the direction and supervision of a licensed Physical Therapist.
• Perform therapeutic interventions to improve patient strength, mobility, and functional independence.
• Assist patients with active and passive therapeutic exercises, muscle re‑education, gait and functional training, transfers, prosthetic training, and activities of daily living.
• Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage as outlined in the plan of care.
• Observe, document, and report patient treatment, response, and progress to the Physical Therapist, Clinical Manager, and Health Care Provider on a timely basis.
• Promptly report significant changes in patient condition, safety concerns, or adverse effects of medical devices.
• Provide care and complete documentation in compliance with State, Federal, Medicare, Medi‑Cal, and third‑party payer regulations and standards.

• Current license as a Physical Therapist Assistant in California.
• At least one year of experience under the supervision of a licensed physical therapist.
• Valid driver’s license and automobile insurance with reliable vehicle.
• Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively and submit in a timely manner.
• Preferred:
  Home Health and Home Care Home Base documentation experience.
