Licensed Vocational Nurse (LVN) - Skilled Nursing Facility Assignment Duration

This is a 13-week contract position.

We are seeking a dedicated Licensed Vocational Nurse (LVN) with experience in a Skilled Nursing Facility (SNF) setting. The ideal candidate will have a solid understanding of Net Health (Rehab Optima) and demonstrate a commitment to providing high-quality patient care.

• Administer medications and treatments as prescribed by physicians.
• Monitor patients' vital signs and report any changes to the healthcare team.
• Assist in the development and implementation of individual care plans.
• Educate patients and their families about care plans and health management.
• Collaborate with the multidisciplinary team to ensure comprehensive care.

• At least 1 year of experience in a Skilled Nursing Facility.
• Knowledge of Net Health (Rehab Optima) preferred.
