Job OverviewThe Nurse Practitioner (NP) is part of the clinical operations team and is responsible for providing direct patient care in Chen Med/Jencare medical centers, nursing homes, skilled nursing facilities (SNF) and home settings. Duties include geriatric assessment, medical history, physical exam, diagnosis and treatment, nursing plan of care development, health education, physician and case‑management referrals, follow‑up, and comprehensive documentation per Chen Med standards.
The NP participates in clinical rounds, conferences, and detailed progress notes.
- Assessment of acute and non‑acute clinical problems; documentation of physical assessments and patient histories; trend analysis and implementation of patient management plans.
- Planning patient care with in‑depth knowledge of the patient population and protocols, considering physiological, psychological, cultural, and support system factors.
- Managing patient care including writing admission, transfer, and discharge orders; ordering and interpreting lab and diagnostic studies; ordering and prescribing medications and treatments.
- Referring patients for specialty consultation as needed (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietitian).
- Documenting in‑depth progress notes and summaries; performing invasive procedures independently upon documented competency.
- Participating in patient care rounds and conferences; communicating patient management strategies; collaborating with the multidisciplinary team to ensure successful outcomes.
- Recognizing situations requiring immediate physician attention and initiating life‑saving procedures when necessary.
- Using advanced communication skills to problem‑solve complex situations and improve processes and patient service.
- Collaborating with multidisciplinary team members to evaluate current healthcare delivery systems and implement new practice patterns.
- Participating in activities that enhance personal and professional growth.
- Initiating arrangements and writing orders for SNF discharges; completing appropriate paperwork.
- Transitioning patients to a lower level of care in collaboration with physicians, nurses, PT, social workers, family, and key caregivers.
- Introducing the NP role to patient and family and facilitating family conferences to review treatment goals and optimize resource utilization.
- Maximizing patient and family decision‑making regarding goals of care, palliative care, and hospice.
- Facilitating discharge to appropriate level of care and using preferred providers for additional services.
- Prescribing medication to patients based on state practice regulations.
- Completing other duties as assigned and modified at the manager’s discretion.

$111,140 - $158,771 Salary (base hourly rate or full‑time salary). Final compensation depends on experience, education, location, and other factors. This position may also be eligible for bonuses or commissions.
