Overview

Join our growing team! Care One Home Health is a locally owned and growing company seeking an experienced RN Case Manager to cover Monterey County and surrounding communities. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. Home health experience ideal, and we will provide training and support. This is a per diem position with potential for full time as we grow.

Requirements
  • Minimum of 1 year of experience as a Registered Nurse with a valid license required
  • Current CPR/BLS Certification
  • Reliable transportation required
  • Experience with Kinnser strongly preferred
  • Comfortable driving to patient homes for patient visits
  • Prior experience with Palliative Care preferred
  • Prior experience in Home Health, Hospice, Infusion, or Home Care preferred
Responsibilities
  • Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate.
  • Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.
  • Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient's home or living facility.
  • Communicate necessary revisions to patient status and/or needs with the entire care team
  • Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.
  • Provide complete, accurate and timely documentation consistent with the plan of care
