- Schedule: Full-time scheduled 40 hours per week
- Shift: Days
- Hours: 8:00 AM – 4:30 PM
- Required Certification: Wisconsin State-issued CNA Certification
- Required License: Wisconsin State issued Driver’s License
- Ability to work independently with strong sense of autonomy
- Previous CNA experience in long-term care or hospital setting

Assists in performing a variety of nursing care services for patients in a home health and hospice setting.
Job Responsibilities and Requirements Primary Responsibilities- Receives patient care assignment from the registered nurse (RN) in charge. Initiates delegated tasks based on demonstrated competence in performing nursing activities for the specific patient age group served.
- Collects and documents data for admission assessments and reassessments. Documents and provides data to the RN and/or LPN for analysis and validation of patient’s response to care or treatment and when significant change occurs in condition.
- Contributes to safety and quality care by relaying information via charting and communicating with team members.
- Informs licensed nurse of any changes in patient condition/needs, patient complaints, and educational needs of patient/significant other.
- Answers patients’ calls for help, takes vital signs, and monitors condition. Assists with baths, ambulating, feeding, skin care, hygiene, and making beds. Performs dressing changes, enemas, ostomy care, toileting, turning, cough and deep breathing, catheter care, collection of specimens, and intake/output measurements.
- Prepares patients for discharge and accompanies them upon dismissal as indicated.
- Stocks and maintains supplies and equipment; uses time and resources efficiently.
- Assists with delegated clerical duties such as answering telephones, relaying messages, transcribing physician orders, and organizing patient records.
- Orients new patients and significant others to the nursing unit.
- Operates equipment per user manuals, recognizes common problems, and initiates corrective action.
- Maintains alertness and follows safety protocols.
- Performs other duties as assigned.
- Completion of an approved nurse aide training program and passing grade on state exam for the work location.
- No experience required
- Proof of motor vehicle liability insurance that meets minimum state requirements upon hire and upon request.
- Constant standing and walking
- Frequent listening, speaking, and interaction to communicate and monitor patients
- Frequent lifting/carrying and pushing/pulling objects weighing 0–25 lbs
- Frequent reaching, grasping, and keyboard use
- Vision requirements for distances near and far, color identification
- Occasional bending, stooping, crawling, kneeling, sitting, and repetitive movements
- Occasional lifting/moving patients weighing 25–50 lbs
- Occasional driving
