In-Home Health - Nurse Practitioner or Physician Assistant; Part Time Oxford ME

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

• Conduct 45‑minute in‑home assessments that include medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry).
• Use an iPad and the intuitive clinical workflow for each evaluation.
• Provide all Signify Health services as indicated, including diagnostic and preventive services products such as spirometry, DEE, and bone density screening.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Maintain patient confidentiality and comply with all applicable regulations.

• Active and unrestricted Nurse Practitioner or Physician Assistant license(s) in the coverage area(s).
• Board certification in the relevant specialty (required).
• Multi‑state licenses (preferred).
• Ability to cover primary locations in Augusta, ME and travel within licensed states.

Anticipated weekly hours: 40. Part‑time. Pay range: $37.66 – $81.11 per hour or base annual salary, depending on experience, education, geography, and other factors.
