LPN​/Licensed Practical Nurse - Home Health

Job Overview

Licensed Practical Nurse (LPN) needed for Home Health services at Adoration Home Health in Montgomery, AL. The position is 8–5, Monday through Friday, covering the local area. This role provides patient‑centered care and supports patients and families in their homes.

• Provide 1:1 care for clients, following individual care plans and maintaining dignity and independence
• Administer medications, feedings, oxygen, and ostomy care as needed
• Accurately document nursing actions, communication, and progress towards established goals
• Coordinate or order supplies, medications, and feedings, ensuring adequate inventory
• Ensure all patient needs meet physician instructions

• LPN, LPT, or LVN degree from an accredited nursing college with current unrestricted license in the state of practice and residence
• Valid driver’s license, acceptable driving record, and proof of car insurance
• CPR professional certification
• 1+ year of nursing experience preferred
• Home health care experience preferred
