We are seeking a compassionate and experienced Nurse Practitioner to join a collaborative care team focused on providing high-quality primary care to senior patients. The NP will provide longitudinal care, build meaningful patient relationships, and help manage patients both in the clinic and in their homes.
Key Responsibilities- Evaluate and treat patients in an outpatient primary care setting.
- Conduct annual health assessments and post-discharge visits.
- Perform comprehensive in-home assessments for identified patients.
- Support chronic disease management and identify gaps in care.
- Perform physical examinations, medication reviews, vital checks, lab draws, wound care, and depression screenings.
- Coordinate care with physicians, specialists, and other healthcare providers.
- Educate patients on disease processes, medications, and treatment compliance.
- Maintain accurate documentation and follow HIPAA requirements.
- Collaborate with Physicians, Care Coordinators, Medical Assistants, and Nurse Care Managers.
- Current and unrestricted Nurse Practitioner license or eligibility to obtain one.
- Graduate of an accredited Nurse Practitioner program.
- Minimum 2 years of NP or PA clinical experience.
- At least 2 years of outpatient Primary Care, Adult Medicine, or Geriatrics experience.
- Experience with risk adjustment, HCC documentation, ICD-10 coding, and Health Risk Assessments (HRA) preferred/required.
- Current DEA certificate required before start.
- Current BLS certification.
- Strong communication, clinical, and patient relationship-building skills.
- Passion for providing care to the senior/geriatric population.
- Full-time primary care position.
