About the Opportunity

We are seeking a compassionate and experienced Nurse Practitioner to join a collaborative care team focused on providing high-quality primary care to senior patients. The NP will provide longitudinal care, build meaningful patient relationships, and help manage patients both in the clinic and in their homes.

Key Responsibilities
  • Evaluate and treat patients in an outpatient primary care setting.
  • Conduct annual health assessments and post-discharge visits.
  • Perform comprehensive in-home assessments for identified patients.
  • Support chronic disease management and identify gaps in care.
  • Perform physical examinations, medication reviews, vital checks, lab draws, wound care, and depression screenings.
  • Coordinate care with physicians, specialists, and other healthcare providers.
  • Educate patients on disease processes, medications, and treatment compliance.
  • Maintain accurate documentation and follow HIPAA requirements.
  • Collaborate with Physicians, Care Coordinators, Medical Assistants, and Nurse Care Managers.
Qualifications
  • Current and unrestricted Nurse Practitioner license or eligibility to obtain one.
  • Graduate of an accredited Nurse Practitioner program.
  • Minimum 2 years of NP or PA clinical experience.
  • At least 2 years of outpatient Primary Care, Adult Medicine, or Geriatrics experience.
  • Experience with risk adjustment, HCC documentation, ICD-10 coding, and Health Risk Assessments (HRA) preferred/required.
  • Current DEA certificate required before start.
  • Current BLS certification.
  • Strong communication, clinical, and patient relationship-building skills.
  • Passion for providing care to the senior/geriatric population.
