Licensed Practical​/Vocational Nurse

Vortex Solutions LLC seeks a Licensed Practical/Vocational Nurse to deliver hands-on clinical support and high-quality client care in a small, service-focused environment. Responsibilities include performing basic assessments, taking vitals, administering medications and treatments per protocols, documenting care accurately, and supporting care plans under RN/physician direction. You'll educate clients, coordinate with external providers, and help refine processes to improve service quality. This role suits an adaptable, accountable LPN/LVN who enjoys autonomy, direct communication, and contributing to a growing, entrepreneurial team.

Responsibilities

• Provide direct patient care including vitals, basic assessments, and routine treatments
• Administer medications and treatments per orders and protocols
• Document all care, observations, and client interactions accurately and promptly
• Support and implement care plans under RN and/or physician supervision
• Educate clients and families on basic health, medications, and follow-up care
• Coordinate with external providers and services to support client needs
• Maintain infection control, safety, and quality standards
• Assist in refining workflows and processes to improve service quality
• Communicate changes in client condition quickly and clearly to leadership
• Participate in training, compliance, and quality improvement activities
