Registered Nurse

Role Description This is a full-time Registered Nurse role providing home health services to patients in coordination with an interdisciplinary care team. The Registered Nurse will perform comprehensive patient assessments, develop and update individualized care plans, and deliver skilled nursing interventions in accordance with physician orders and regulatory standards. Daily responsibilities include monitoring patient status, administering medications and treatments, providing patient and family education, documenting clinical findings in the electronic health record, and communicating changes in condition to providers.

The nurse will collaborate closely with other clinicians to support safe transitions of care, promote adherence to care plans, and advocate for patient needs while maintaining accurate, timely documentation and participation in case conferences.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Alabama (or ability to obtain), and current Basic Life Support (BLS) certification.
• Experience in home health, community health, or related clinical settings, with strong skills in patient assessment, care planning, and clinical decision-making.
• Proficiency in electronic health records, remote documentation, and use of telehealth or virtual communication tools.
• Strong communication and interpersonal skills to educate patients and families, coordinate with care teams, and build trust in diverse communities.
• Ability to work independently in a remote environment, manage time effectively, prioritize multiple cases, and maintain confidentiality and compliance with all regulations.
• Knowledge of Medicare, Medicaid, and home health regulatory requirements is beneficial.
• Preferred qualifications include prior home health RN experience and additional certifications in areas such as wound care or chronic disease management.
