Licensed Practical Nurse; LPN

• Compensation: USD 24.3 - USD 27 - hourly

Use your nursing experience to help patients navigate specialty care in a military healthcare environment, from initial referral through appointment coordination and completion of care.

As a Licensed Practical Nurse (LPN) supporting referral management services, you’ll review and coordinate specialty care referrals, educate patients about next steps, and collaborate with clinical teams to promote timely access to appropriate care.

You’ll work across electronic healthcare systems to manage referrals from initiation through closure, maintain accurate documentation, track clinical results, and identify patients who may benefit from additional care coordination.

Compensation:

• $24.30–$27.00 per hour
• Up to $5.09 per hour in Health and Welfare benefits, in accordance with the SCA

Schedule: Monday–Friday, 7:30 a.m.–4:30 p.m. The position does not work federal holidays or designated facility closure days.

Key Responsibilities

• Review specialty care referrals for clinical appropriateness, administrative completeness, covered benefits, and required tests or preparation.
• Coordinate referrals and appointments with patients, referring providers, specialty clinics, and other healthcare partners.
• Educate patients about referral procedures, laboratory requirements, specialty appointments, diagnoses, and available healthcare benefits.
• Initiate, monitor, and close referrals within established timelines while maintaining accurate documentation in applicable healthcare systems.
• Track consultation results, incorporate records into the appropriate medical record, and return results to referring providers.
• Identify patients who may benefit from case management or alternative levels of care and coordinate with the appropriate clinical team.
• Collect and analyze referral data, support quality and process-improvement activities, and provide oversight of referral management staff and processes as directed by the onsite quality representative.

Required Qualifications

• Graduate of an accredited practical or vocational nursing program approved by the appropriate state agency.
• Current, valid, and unrestricted nursing license in good standing, with no clinical restrictions in any jurisdiction where a license is currently or has been held within the past 10 years.
• Current Basic Life Support certification through the American Heart Association or American Red Cross.
• At least two years of broad-based clinical nursing experience in an inpatient or outpatient setting within the past three years.
• Knowledge and computer literacy sufficient to interpret and apply medical-care criteria such as Inter Qual or Milliman Ambulatory Care Guidelines.
• Working knowledge of Microsoft Windows, Word, Excel, and Outlook, along with strong medical ethics, customer service, telephone etiquette, and written, verbal, and electronic communication skills.
