Tri Health is seeking a Diabetes Care & Education Specialist to deliver comprehensive diabetes education and medical nutrition therapy. The role combines in-person care at the West Chester and Montgomery offices with one remote day per week to support patients across the care continuum.
The position requires a Bachelor’s degree in nursing or Dietetics/Nutrition, RN or RD licensure, and 2–3 years of experience, plus CDCES eligibility or certification within 24 months.
