- Thursday.
8 a.m. to 12:00 pm on Friday.
However, additional hours may be required to complete duties.
POSITION SUMMARY:The Health Coach is part of the care team andunder the supervision of the Director of Health Education. The Health Coach will work in conjunction with the Diabetes Self-Management team to assist the care team with direction and resources regarding diabetes self-management aimed at meeting the needs of the patients. The health coach will integrate clinical and self-management aspects of diabetes and cardiometabolic care in order to optimize quality care.
The health coach promotes self-management to achieve individualized behavioral and treatment goals. The Health Coach will assist in meeting organizational goals. The Health Coach is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.
DUTIES AND RESPONSIBILITIES:
- Coordinate communication between patients, families, and caregivers with the multidisciplinary care team in order to identify barriers and support care plan.
- Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions and self-management of diabetes into clinical practice.
- Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of pre-diabetes, diabetes and cardiometabolic conditions across the lifespan.
- Provides quality diabetes self-management education in individual and group settings based on assessed needs. Uses appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.
- Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.
- Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’s assessed needs and goals and promotes successful self-management.
- Documents all individual contacts/visits in the EHR according to MHSI and Accreditation Program guidelines in a timely manner.
- Ability to properly process data and compile reports on various projects.
- Notification and recall of patients regarding missed appointments and care gaps.
- Ability to communicate with patients and staff daily in a courteous and professional manner, address questions and direct calls promptly.
- Serve as part of the care team in developing person-centered diabetes plans; follows and implements workflows that support and sustain MHSI Patient Centered Medical Home model of care.
- Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.
- Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
- Collaborates with community partners to establish and maintain on-going support options.
- Participates in all Risk Management activities as deemed appropriate.

Education:
- Associates Degree in Nursing Required or Bachelor Science in Nursing
