Salary: $55,000 - $60,000 depending on experience
- Skilled Nursing Care
:
Ensures provision of skilled nursing care to students including but not limited to triage of and first aid to ill/injured students; accurate administration/self-administration of medications to and by students; implementation of ordered care plans; and assessment and screening for student health risks and medical conditions. Ensures the safety of individuals attending school clinics. Identifies students at risk for pregnancy, STDs, substance abuse, and social and behavioral health issues.
Refers those students in need of further care. - Collaboration
:
Establishes and maintains effective working relationships with Jefferson County Schools (JCS) students, parents, and faculty; DOH Jefferson team members; and DOH School Health program personnel. - Parent/Guardian Communication
:
Ensures communication with students’ parent/caregiver promptly and often, including but not limited to failed screenings; referrals and their processes; medical concerns, issues, or updates; medication re-supply request; and IMMEDIATE notification of injury or suspected injury. - Care Plans
:
Provides individualized healthcare plans and emergency care plans for students with applicable health conditions according to established policy, guidelines, and protocols. Identifies and decreases barriers to compliance through parent/caregiver communication, education, and resource awareness. - Documentation
:
Monitors all clinic records and documentation for accuracy. This includes but is not limited to clinic logs, medication logs, electronic records, and paper health records. Maintains proficiency in JCS and DOH software. - Screenings
:
Plans, coordinates, and implements all mandated health screenings by assigned dates as set forth by the School Health Program. Screenings include but are not limited to vision, hearing, body mass index, and scoliosis. Identifies and completes re-screenings and medical evaluation referrals for students. - Education
:
Coordinates and implements health education presentations to students. May also provide health education to JCS staff and community as requested or directed. - Infection Control
:
Adheres to universal precautions. Triages all students for infection risk, and isolates those students identified as potentially infectious. Ensures school clinics remain clean and tidy. Ensures all clinic surfaces are sanitized at the end of each day.
- Reporting Deliverables
:
Prepares, reviews with supervisor, and submits by assigned date: annual School Health Report, biannual School Health Services Plan, and Kindergarten and Seventh Grade Compulsory Immunizations Annual Report. Completes internal monthly Leadership Reports and quarterly Performance Management Worksheet. Completes additional reporting as specified by supervisor or senior leadership. - Meetings
:
Attends and engages in all assigned meetings. These include but are not limited to: internal staff and leadership meetings, statewide programatic meetings, school meetings, and collaborative partner meetings. - Data Compliance and Review
:
Ensures student records are reviewed, complete and include up to date immunization records and physicals by assigned dates set forth by the DOH School Health Program. Enters client service records (CSRs) in Employee Activity Records (EARs) into Health Management System (HMS) within 7 days of the service date, or the date of receipt of service data, in accordance with the current School Health Coding Manual, DOH coding manual, and DOH-Jefferson performance requirements. - Supply Management and Budget
:
