Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Responsibilities- Deliver face-to-face primary care services and participate in educational series and clinical reviews.
- Collaborate with Nurse Practitioners and Physician Assistants.
- Work in a structured care model aimed at improving clinical, cultural, and financial outcomes for partner facilities.
