THIS ROLE IS SIGN ON BONUS ELIGIBLE. The position is full‑time and requires a primary residence in the state of New Hampshire.

Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role requires traveling up to 100% of the time across a defined coverage area in New Hampshire and potentially other states. The clinician will perform physical assessments, review medication and medical history, and conduct diagnostic tests as ordered by the health plan.

• Conduct in‑home health evaluations lasting approximately 45 minutes, including a medication and medical history review, physical assessment, and diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered.
• Use an iPad and an intuitive clinical workflow for each evaluation; not required to prescribe medicine, order lab tests, or alter current treatment regimens.
• Travel locally and within licensed states up to 100% of the time, including overnight hotel stays, car rentals, and flights as required by the coverage area.
• Provide virtual visits as needed.
• Perform Diagnostic & Preventive Services products such as spirometry and diabetic eye exams.
• Work collaboratively with Signify Health’s clinician colleagues and the network activation team.
• Maintain a safe and comfortable environment in patient homes, which may contain pets, secondhand smoke, or other allergens.

• Active, unrestricted license(s) in the coverage area(s) (New Hampshire and, as required, additional states).
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses as required.
• Coverage of all counties within New Hampshire with flexibility to cover other areas of business need.
• New Hampshire Coverage Market Areas include:
  Rockingham, Strafford, Belknap, Merrimack, Sullivan, Coos, Carroll, Grafton, Cheshire, Manchester, and Hillsborough.

Pay Range: $87,035.00 - $ (base hourly or base annual salary, depending on experience, education, geography, and other factors). Additional per‑completion incentive pay is provided for all in‑home visits.

Anticipated weekly hours: 40. Full‑time position.
