The University of Vermont Health Network seeks an Office Licensed Practical Nurse II to deliver high-quality, patient-centered care in our Healthcare & Medical Services department. In this ambulatory role, you'll perform assessments, administer medications and immunizations, support procedures, coordinate care, and provide education for diverse patient populations. You'll work collaboratively within an academic, evidence-based environment that values professional growth, inclusion, and service to our Vermont and northern New York communities.
Responsibilities
- Perform patient intake, assessments, and vital signs for diverse populations in an ambulatory setting
- Administer medications, vaccines, and treatments per provider orders and organizational protocols
- Support minor procedures and adhere to strict infection prevention and safety standards
- Document accurately in the Electronic Health Record and assist with care coordination and follow-up
- Provide patient and family education to support understanding of diagnoses, medications, and care plans
- Triage patient calls and in-clinic concerns, escalating to providers as needed
- Collaborate with physicians, RNs, and interdisciplinary team members to deliver evidence-based care
- Participate in quality improvement, population health, and practice workflow initiatives
- Maintain licensure, required competencies, and ongoing professional development
- Contribute to an inclusive, respectful, and patient-centered team culture
