Job Overview
Come join our awesome team as a RN with Horizons Care Center! Horizons Care Center is part of Volunteers of America National Services, the Housing and Healthcare subsidiary of the Volunteers of America parent organization. The position is located at the base of the scenic Grand Mesa in Eckert, CO. Pay ranges from $37.00 to $43.50 per hour based on experience, plus a $2.75 per hour night shift differential.
Full‑time nights are 6 pm to 6 am, 72 hours per pay period.
- Leadership and Unit Management
- Provide leadership and direction to the assigned unit.
- Role model positive communication skills with nursing assistants, CNAs, STNA’s, TMA’s, interdisciplinary team, supervisory and management team, customers, residents and families.
- Listen to shift reports, keep open lines of communication with supervisor, co‑workers and incoming shift.
- Make rounds to residents to observe their physical and psychosocial status.
- Assign personnel appropriately to meet resident needs.
- Ensure unit personnel on your shift carry out their assignments.
- Attend nurses and team meetings as scheduled and/or assigned.
- Initiate disciplinary action when warranted.
- Report occupational exposures (needle sticks, blood, body fluids, infectious materials, hazardous chemicals) in accordance with facility policy and procedures.
- Present concerns and improvement suggestions to facility leadership and QAA/QAPI committee.
- Resident Care Functions
- Maintain competence in all required skills within assigned area(s).
- Provide nursing techniques for comfort and well‑being of residents.
- Administer prescribed medications and treatments per MD orders.
- Monitor resident response to drugs, side effects, and report problems or potential problems to physicians.
- Maintain accurate and timely documentation per policies and procedures.
- Follow shift routines, monitoring vital signs, weights, medication administration, and emergency protocols.
- Make physician/provider calls to report resident change in condition.
- Keep resident representatives and families updated on resident conditions.
- Follow advance directives: DNR/DNI/CPR/Comfort Care protocols.
- Charting, Documentation and Assessment
- Contribute toward completing and updating resident’s care plans.
- Perform routine charting duties as required in accordance with established policies.
- Accurately transcribe physician’s orders and forward necessary information.
- Document and report all incidents, accidents involving residents, visitors, and staff.
- Chart nurses’ notes in an informative, descriptive manner reflecting care provided and resident response.
- Provide ongoing resident assessments as necessary using facility policy and E‑Interact COC & Care Paths.
- Personnel Functions
- Attend scheduled nurses meetings.
- Maintain a clean, safe, efficient work environment.
- Maintain continuing education requirements.
- Participate in employee performance evaluations and staffing recommendations.
- Develop and maintain working relationships with inter‑departmental personnel.
- Attend required in‑services and complete assigned online modules.
- Committee Functions
- Serve on, participate in, and attend various committees and meetings to enhance interdisciplinary processes.
- Other Duties
- Perform other duties as assigned.
- Registered Nurse licensure in the state of Colorado.
- Current CPR certification.
- Maintenance of required CEU’s.
- Experience in long‑term care preferred.
- Good problem‑solving skills with independent judgment utilizing the nursing process.
- Demonstrated leadership ability.
- Effective communicator with staff, families and outside agencies.
- Ability to prioritize duties.
- Customer service orientation and knowledge of successful techniques.
- Clinical competence for physical and cognitive assessments of residents.
- Computer and electronic device proficiency for timekeeping, in‑servicing and documentation.
- Team player, understanding of antibiotic stewardship, infection prevention and control, and OSHA guidelines.
