Senior Community Care is seeking an LPN to provide hands-on nursing care for seniors enrolled in our PACE program. The LPN will administer medications and treatments, monitor vitals and chronic conditions, support provider visits, and document in the EMR. This role coordinates with RNs, therapists, social workers, and providers to deliver holistic, person-centered care that supports seniors living safely in the community.

The LPN will educate participants and families, promote dignity and independence, and contribute to quality and safety initiatives in a team-focused environment.
