LPN, LPN​/LVN, Nursing Home

Valley Manor Care Center, part of Volunteers of America National Services, seeks an LPN to provide compassionate, resident-centered care in a skilled nursing and rehab community. The LPN delivers hands-on clinical care, including medication administration, treatments, assessments, and documentation, while supporting residents with daily living needs. Working within a faith-based, mission-driven team, the LPN collaborates with RNs, therapists, and caregivers to promote dignity, safety, and holistic well-being for seniors, contributing to quality outcomes and meaningful relationships every day.

Responsibilities

• Provide direct bedside nursing care to residents under supervision of an RN or provider
• Administer medications and treatments per care plans and document accurately in electronic health records
• Monitor residents' conditions, perform assessments, and promptly report changes to the RN or provider
• Assist with ADLs, mobility, and restorative care while promoting resident dignity and comfort
• Collaborate with interdisciplinary team to develop and update individualized care plans
• Educate residents and families on health conditions, medications, and care routines
• Follow infection control, safety, and regulatory standards in all care activities
• Participate in care conferences, shift reports, and quality improvement initiatives
• Support a mission-driven, faith-based, resident-centered environment
• Mentor and support CNAs and other team members as appropriate
