Star Light Home Health in Montrose, CO is looking for a passionate and driven Assistant Director of Nursing to join our dedicated care team. Responsibilities- Assist the Director of Nursing in overseeing daily clinical operations and ensuring high-quality patient care
- Supervise, mentor, and support nursing staff and field clinicians to promote best practices
- Conduct patient assessments, care plan reviews, and ensure compliance with home health regulations
- Monitor and evaluate clinical outcomes, identifying areas for quality improvement
- Ensure adherence to Medicare, Medicaid, and state/federal home health guidelines
- Participate in hiring, onboarding, and ongoing education of nursing personnel
- Collaborate with physicians, therapists, and interdisciplinary team members on patient care coordination
- Active RN license in the state of Colorado (or compact license eligibility)
- Minimum 2-3 years of clinical nursing experience, preferably in home health or community care
- Prior supervisory or nursing leadership experience strongly preferred
- Strong knowledge of Medicare/Medicaid home health conditions of participation
- Excellent communication, organizational, and critical thinking skills
- Experience with OASIS documentation and home health EMR systems a plus
