Job DescriptionThe Occupational Therapist is responsible for screening, evaluating, and developing care plans for patients with cognitive and/or ADL physical functioning disorders. The role requires effective communication and collaboration with patients, families, caregivers, and other service providers.
Responsibilities- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders.
- Develop appropriate plans of care adhering to regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment interventions.
- Administer various rehabilitation procedures including ADL retraining, therapeutic activities, and exercises.
- Document all required information in the electronic documentation system and maintain patient records.
- Assess patient progress and adjust treatments based on evaluation outcomes.
- Provide consultation and counseling to patients, families, and caregivers.
- Participate in patient care conferences, utilization review meetings, and required trainings.
- Supervise occupational therapy assistants and rehabilitation aides or students.
- 1+ years of Occupational Therapy experience (REQUIRED).
- Experience in Long Term Care (LTC) or Rehabilitation (REQUIRED).
- Active Occupational Therapy license (REQUIRED).
- New graduates considered with Level II experience, not just Level I observation.
