We are seeking a dedicated Physical Therapist to effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care. The successful candidate will develop appropriate plans of care while adhering to all regulatory and clinical practice standards.
Responsibilities- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Administer various therapeutic procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
- Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations.
- Consult with patients, families, caregivers, and other service providers regarding physical disorders.
- Communicate effectively with supervisors and health team members about patient progress and treatment plans.
- Provide comprehensive discharge summaries in compliance with regulatory and clinical requirements.
- Document all required information accurately in the electronic documentation system.
- Participate in required meetings and trainings, including patient care conferences and rehabilitation meetings.
- Supervise and support physical therapist assistants and rehabilitation aides as needed.
- Adhere to APTA and state-required practice acts and ethics.

ScheduleThis is a Days position, requiring 8‑hour shifts, typically Monday through Friday.
Required Experience / Certifications / Licensure- Minimum of 1 year of experience as a Physical Therapist is required; new graduates with a Level II clinical experience will be considered.
- Experience in Long‑Term Care (LTC) or Rehabilitation is required.
- Current Oklahoma State Physical Therapist license is required.
